The Billing Department handles claims from inside the clinic, any claims from outside of the clinic are handled through the Contract Health Department.

Insurance Questionnaires: If you receive a questionnaire in the mail from your insurance company please complete and return back to your insurance company so your claims can be processed.

AFLAC: Requests for Date of Service Information for AFLAC are processed on Thursdays. An AFLAC release is needed with the Patient Date of Birth and Date of Service. Pickup AFLAC at the Cashier Window. **If patient is 18 years of age or older, they must sign and pickup their own paperwork. This is a HIPAA requirement for release of patient information.** To request AFLAC please call 715-799-5484.

New Medical Assistance Non Native Patients: Per MTC Policy, we are currently not accepting any new non native medical assistance patients in our Dental or Optical Departments. Definition of new patient: If you have not been seen at the Menominee Tribal Clinic within the last 3 years, you are considered a new patient.

Methods of payment include: Cash, local checks, Visa, MasterCard, and Wage Assignments (for those with employment within the Menominee Tribe).

We wouldn’t treat medicare coverage different from any other facility. A non Indian would have to pay the co-insurance of 20% and a Native American patient would receive an adjustment on their 20% because we are an Indian Health Service (IHS) facility.

Yes, our policy is 10% of the current balance of the patient due amount with a minimum monthly payment of $30 a month.

Yes, as a benefit to the patient we will file the insurance claim.

Yes, simply inform your doctor who is providing the service to you.

If you’re a medical assistance patient, you will be required to pay the co-payment before your appointment. (After you register you will be sent to billing.)
